Dementia Support Programme
A UNIQUE SUPPORT SERVICE, DEDICATED TO LEADING THE WAY IN DEMENTIA CARE
The Carewatch Dementia Support Programme, offers a unique service in South East Essex, to all People that may be affected by Dementia, in their own home. The service is specifically aimed at meeting the individual needs of People with Dementia, and their carers and families.
A team of care workers dedicated to the Dementia Support Programme provide a flexible and responsive service, which may be in addition to an existing care plan, or as a stand alone service.
The Team are trained to understand the four stages of Dementia, and the impact that these have on the Person with Dementia and their family; they can assist to develop coping strategies that are unique to that person, and their family and friends.
Our Dementia Champions can provide one to one support to the informal carer and the family network, and can assist to signpost toward available funding , such as Carers Direct Payments, and the fully funded Respite* and Prescription Break** Service

A team of care workers that can provide respite at home for a few hours, allowing the carer time to visit the shops, attend appointments, play bowls, go to the hairdressers, visit family members
